Question to ladies who love their Men - Regarding Penis Sleeves or Extensions?

I'm a 26 yrs old male my sex life was excellent, me and my wife are sexually active partners. Nowadays, I'm experiencing problems during/after sex. I love my wife a lot and will do anything to satisfy her sexually, but I feel pain in my testicles and lower back when I ejaculate and after ejaculation my left leg feels weak and nerves tingle - I also feel that I forget things quite easily after sex and I get lazy for the whole day after ejaculation. My urologist told me that I need a break - a long break from ejaculation. I was just thinking how would I be able explain that to my wife. I really love her and I really feel sad and ashamed to approach her regarding this matter and I see its like a challenge for my manhood. My doctor told me that I could use a penis sleeve, extension and an anesthetic cream that wouldn't make me feel anything but satisfy my wife. I just want an honest advice on how should I bring this topic up with my wife? and how should I convince her on this issue?. And if your husband/boyfriend wants to use such a device how would you feel?.
